We have observed the increasing use of Maryland Emergency Departments ED ; by patients suffering from psychiatric disorders. In the situation described below, what seemed like routine evaluation and treatment of a patient known to suffer from a psychiatric disorder turned into a patient nearly dying because of the failure of a very basic responsibility on the part of Emergency Department staff to properly, quickly, and effectively search a patient with behavioral health concerns. Patient A was brought by police to a Maryland ED on an Emergency Petition because she was delusional and agitated. She was known to the staff from previous visits, and had never before shown any suicidal or homicidal ideation. She was evaluated in the main ED, treated with Geodon an anti-psychotic medication used for acute agitation ; , medically cleared and sent to an area adjacent to the ED where psychiatric patients are taken for stabilization and evaluation before being admitted to an inpatient psych unit. Her toxicology screen was positive for PCP, cocaine, and THC. She was quickly evaluated by nursing staff and a psychiatrist and then medicated with Haldol and Cogentin an anti-psychotic and a medication to reduce the side effects of the anti-psychotics ; . She had arrived in 4-point restraints. After being medicated, the RN removed the ankle restraints, leaving the wrist restraints on. The patient was placed in a private room directly across from the entrance to the nurse's station, with the door open and a camera operating, although no one was actively watching the monitor. After a very brief amount of time, a nurse tech smelled smoke and found patient A on fire. The patient had apparently been able to put her hand in her pocket, and light her cigarette lighter. The patient suffered 2nd and 3rd degree burns of the left flank, leg, chest, arm, and hand. She was sedated and intubated, and sent to the burn center of another hospital. A subsequent investigation by the hospital revealed that the patient was restrained in her street clothes and had not been searched, either in the main ED or in the adjacent psychiatric evaluation area. The hospital's policy regarding searching psychiatric patients did not specify when the search should take place, or who should do the searching-- nursing staff or Security. The lack of a coherent and specific policy regarding searching and retention of clothes by psychiatric patients contributed to this very unfortunate outcome Could this happen in your hospital ED? Is there a policy in effect, with properly trained staff, to ensure that this kind of error could not occur?.

Bangladesh, Bhutan, India, Myanmar and Nepal have a total population of 1.19 billion, of which 1.01 billion i.e. 92.4% are residing in malaria risk areas. There are considerable inter and intra-country geographical variations in the relative incidence of malaria. Malaria-free areas comprise the temperate regions of Himalayas, snow capped mountains in Papua, Indonesia and the Islands of Maldives. Approximately 2.48 million malaria cases are reported annually from South-Asia, and among them 1.06 million are P falciparum cases, showing a high falciparum rate of 42.7%. P falciparum is associated with malaria outbreaks and malaria-related mortality, and the parasite has acquired resistance to standard antimalarial drugs. Drug-resistant malaria also constitutes a major problem in cross-border malaria areas, where there is substantial migration, to and from countries within the SEA Region. Since drug resistance enhances malaria morbidity and mortality, it has become an important crucial problem in malaria control. The association of drug resistance with population movement, infrastructure deficiencies, drug pressure, haphazard use of drugs and the flourishing market of counterfeit drugs have aggravated the malaria situation requiring immediate remedial measures. To tackle the problem of drug resistant malaria in the Mekong region, six countries of the Region held a meeting in Phnom Penh, Cambodia in October 2000. Member Countries developed a network to monitor drug-resistant malaria and identified factors contributing to its emergence and spread in Mekong region. This information is critical to ensure that malaria diagnosis, treatment guidelines, and drug policies are appropriate and based on current quality data. The programme on monitoring of drugs in the Region is progressing satisfactorily. In South-Asia SA ; , information on drug-resistant malaria is not routinely collected nor is data shared between the Member Countries. Considering the increasing trend of mono and multi-drug resistance, the establishment of coordinated network for monitoring drug-resistant malaria in the SA region is and ismo.

Loss that were caused by nonarteritic anterior ischemic optic neuropathy NAION ; .37 In a notice to health care professionals, the FDA noted that most of the affected patients had underlying anatomic or vascular risk factors for NAION. The FDA also stated that too little evidence existed to determine whether use of PDE5 inhibitors or any other specific factors caused the vision problems. The FDA advised physicians to tell patients to stop PDE5 inhibitors immediately in the event of sudden vision loss in 1 or both eyes. The FDA also recommended that physicians discuss with patients the increased risk of NAION in people who have already experienced the problem in 1 eye and the possibility that use of a PDE5 inhibitor could have adverse effects in people with a history of NAION and imipramine and haldol, for example, haldoll and pregnancy.

Three drugs commonly prescribed for schizophrenia and other psychotic illnesses increased patients' risk of developing diabetes when compared with older antipsychotic medications. The drugs Zyprexa, Risperdal, and Seroquel were associated with higher rates of diabetes than older generation drugs for schizophrenia like Haldol. But the increased risk was statistically significant only for Zyprexa and Risperdal, possibly because of the smaller number of subjects who took Seroquel. Younger patients, under aged 54, who took Zyprexa or Risperdal, showed the highest risk of developing diabetes, the study found. The results add to a growing number of reports linking Type 2 diabetes to some drugs in the class of antipsychotics known as atypicals. "These findings are absolutely consistent with everything we've looked at and seen, " said Robert Rosenheck, a professor of psychiatry and public health at Yale and an author of an earlier study that found an increased risk of diabetes with Zyprexa, Risperdal, Seroquel and Clozaril. Experts said the new findings underscored the need for patients who take the drugs and doctors who prescribe them to be alert for the symptoms of diabetes, including increased thirst, frequent urination, increased appetite and rapid weight gain. Atypical psychotics are less likely than older drugs to produce effects like tardive dyskinesia, a devastating movement disorder. The newer drugs also appear more effective in preventing relapse in patients with schizophrenia and may be more effective in treating certain aspects of the illness.

There are many examples from the Silver Screen where a song has revived memories for two lovers. On a more mundane level this may also occur in patients with amnesia Neurocase 2003; 8: 4535 ; . Follow up of two amnesic patients with brain damage established that if songs were played to them they retained their general content. A similar phenomenon may be the aphasic patient who can sing words or generate them when under emotional pressure and haloperidol.

While it is crucial that specific medication recommendations be made by a physician psychiatrist, pediatrician, neurologist ; , some general direction can be given. Medicating for tics is not usually needed for the average case of TS which is mild ; , or when the predominant problem is lack of understanding or comfort. In these situations, psychoeducation for those around the child is usually the better treatment. When medication IS required, and clonidine as described above ; is insufficient, atypical antipsychotic medications such as Risperdal ; or classic neuroleptics such as Haldol ; may also be tried. Various other medications e.g. amantanine, tetrabenazine ; are currently being studied by our clinic and others. With regards to ADHD, research at this time is clear and definitive that the best treatment for the hyperactivity and impulsivity in ADHD is a medication either a stimulant medication such as Ritalin or a non-stimulant such as Strattera ; . Finally, medicating for OCD is very much an individual choice: while TREATMENT is usually indicated, both medicinal generally, the use of a selective serotonin reuptake inhibitor or SSRI ; and non-medicinal exposure & response prevention or ERP ; treatments can be equally effective depending upon individual factors, with ERP shown to have more long-lasting effects.
For continuous prevention, a few people who have frequent outbreaks can only control the outbreaks by taking medication every day.
